INTRODUCTION Lutech is about to take up construction of New storage tanks as per API 650 for storage of petroleum products, namely, TK 8 (Crude), TK 9 (Naptha), TK 10 / TK 11 (both Naptha), TK 12 / TK 13 (both AGO), TK 14 / TK 15 (HFO). Two more Service Tanks will be constructed on HFO and AGO service namely, Tk 16 and Tk 17 respectively, necessary piping to Tanker discharge line header, BRV loading gantry, using the existing Pump house area, necessary civil construction for Refining facility like CDU, Boiler, Heat Exchanger, Furnace etc., integrating Tank Farm / Pump house with Refining facilities and help erecting refinery facilities under the supervision and guidance of M/s. V-Fuels, USA as per drawing / lay out, for M/s. Conex Petroleum Refining Coy for their proposed 10,000 BPD Petroleum Refining at Free Port, Monrovia. There would be additional activities like integration of New facility as stated above to existing Conex PST Terminal that includes cleaning of TK 5 & TK 6 and related tie in work after tank cleaning and gas freeing pipeline and Tanks cleaning done by Conex PST. There are plan to tie in pipeline of Naptha and Gas Oil from ISBL to OSBL to that with Mo Gas and Gas Oil Shipping line respectively to facilitate Export. Shut down shall be required for integration of the existing facilities with the new facilities which can be done on holidays based on the schedule to be provided by Conex. The project poses a challenge in terms of Health, Safety and Environment as the construction has to be carried out in an existing oil terminal without disrupting day to day operation of Conex PST terminal. Considering the above, Lutech Engineering management has decided to develop an Emergency Response Plan to meet any Emergency situation arising out of Construction and related activities, despite having a sound Site Safety, Health and Environment Plan as well as practices.

This document explains the Basic Philosophy, Policy, Actions needed and Methodology thereof to contain Injuries, loss of properties and damages to environment. Imperatives of such a plan are to minimize the adverse effects of an accident and restoration of normalcy at the earliest by providing measures to contain the incident and limit the consequences. Above objectives are sought to be achieved through following measures: •



Providing information to all concerned authorities regarding consequences of likely emergencies.







Warning and advising Site Engineer, Staff, Sub Contractor staff, plant personnel & people likely to be affected.







Mobilizing on-site and off-site resources.







Calling up assistance from TM, CONEX PST / outside agencies.







Initiating and organizing evacuation of affected people.







Providing necessary First aid and other medical services as required







Coordinated action taken by various agencies.



This Emergency Response Plan adopted will go a long way to control and contain Injuries, loss of properties and damages to environment, in the event of an Incident, under various circumstances. The Lutech Engineering is committed to this Emergency Response Plan to put into Practice in letter and spirit.

GENERAL RESPONSE PROCEDURE It is expected that Lutech Engineering Project site has an established and wellrehearsed plan of responses to an Emergency situation which may arise out of various activities undertaken. It is our intention to provide guidance to make the Emergency Response Plan effective. •



General Response Procedure Site Management shall establish and train site personnel regarding emergency response procedures. Site Management shall maintain, as necessary, emergency response supplies and equipment to meet emergency response needs. The Engineers / Supervisors will notify emergency response personnel of emergencies at the project site. The Construction supervisors will notify emergency response personnel of emergencies at the project site. The appropriate supervisor or responding personnel shall initiate the notification process, which includes alerting Project Manager and or Terminal Manager of Conex PST under whose jurisdiction Project work being undertaken who in turn shall inform Ambulance, Port Authority / Police, Fire Service and / or others as required. Notify the following immediately: o Project Manager o Project Quality & Planning Manager o Project HSE Manager / HSE Supervisor The Project Manager will also inform Project Director about the Incident.

Emergency Preparedness Training Employees and subcontractor management and employees shall be trained on the subjects below as appropriate: o o o o

Emergency Action Plan Project Management will ensure the Emergency Action Plan is communicated to all workers during orientation. Specific emergency procedures and emergency phone numbers will be posted in site Office, near all telephones and on project bulletin boards. The plan shall be reviewed periodically by Lutech Engineering to ensure continued accuracy and applicability. Daily Pre -Task Plans shall also address emergency egress on a daily basis from each work area. This is a project specific Emergency Action Plan communicating evacuation procedures, specific alarms, and assembly points, should an emergency evacuation become necessary because of severe weather, fire, hazardous chemical release, explosion or other emergencies that could cause worker harm. The Emergency Exit Route, Assembly Points (Primary and secondary) will be clearly marked. It is each worker’s responsibility to familiarize themselves with evacuation routes, alarms and assembly points in case an emergency evacuation of the work area is required.

The Role and Responsibilities of all stakeholders including Project Management, Sub contractor Supervisor / worker will be stated in detail Emergency Response Plan document which will be integrated with Terminal Emergency Response Plan. •



Evacuation Exit signs shall be conspicuously posted along evacuation routes. A signal or alarm shall be designated to initiate evacuation. Personnel should de-energize / shut down tools and equipment and observe their work area for fellow workers in need of assistance. Observe stairs for safe passage before accessing. Medical Emergency During the safety orientation, workers will be given information on how to summon medical assistance in case of a medical emergency. Workers should know the following information:

When reporting a medical emergency, the worker will state their name, the nature of the emergency, the severity of the emergency and where assistance is needed. A worker may be required to meet medical personnel and guide them to where the emergency is located. Severe weather Should weather conditions such as Cyclone, severe thunderstorms, torrential rain develop around work site or near the Project, Workers will follow the direction of their Supervisor. At Monrovia, such weather condition does not prevail, other than heavy rain during monsoon.







Petroleum / Chemical Release or Explosion Workers will immediately evacuate their work area upon hearing the alarm or being notified of the emergency and ordered to evacuate. No worker is exempt from evacuation even if the evacuation is a drill. Workers are required to report immediately to their designated assembly point and be accounted for. Failure to report may cause another to risk danger in an effort to search for missing worker. Do not leave the project without prior authorization from front-line supervision. A Lutech employee will call for assistance from Terminal Manager. Any other Chemical or Gas leakage or spillage will be identified in PMP

Incident & Injury Management and Reporting Policy: In order to control and manage any incident on a Lutech Engineering project the following measures will be followed: o Lutech Engineering Supervision shall take control of the administrative management of the incident. If a subcontractor is injured, Lutech Engineering reserves the right to appoint a supervisor from the subcontractor to help keep track of the injured person until a full release to work can be obtained. Training will be completed with the supervisor of the subcontractor to insure the best care be given to the employee. o Each project will have Lutech and subcontractor personnel on-site during all work activities that are trained in first aid and Cardiovascular Pulmonary Resuscitation (CPR). o All injuries will be reported to Lutech and subcontractor management immediately. Written reports will be submitted to Lutech management during the same shift. o Subcontractors will submit a copy of a First Incident Report of Injury form (from the Doctor to Lutech Management. o In the event of an injury or property damage accident / incident, the applicable Contractor Representative shall contact the Lutech Engineering Supervisor to obtain an Incident Packet that will contain all of the applicable forms and literature, including: ▪ ▪ ▪

o All employees working on Lutech Engineering projects will follow the Return to Work Policy in this manual. Each subcontractor shall be responsible for ensuring their employees comply with this manual. Light duty is a mandatory requirement on each project to help in the quick recovery of the employee. Subcontractors will establish or follow the LUTECH Light Duty program. All means necessary will be provided to the injured employee to ensure a rapid recovery and return to work processes. As a part of this plan, the injured employee will be provided the opportunity to return to work as soon as possible on light duty and transition to full duty upon medical full release from care. Lutech Engineering, along with the subcontractor supervision, will work closely with all medical doctors, and specialists in order to ensure close co-operation from all parties in returning the injured employee back to full and normal duties as soon as possible. Lutech Engineering shall monitor all medical and other costs involved from the injury to minimize and control the cost of providing necessary medical treatment. As soon as an injured person has been removed from the scene, have someone immediately secure the scene without disturbing physical evidence and take names and phone numbers of all witnesses. If necessary, administer first aid until help arrives. Strategically place employees to direct the emergency response team to the incident. For emergencies requiring evacuation, each project will develop a site-specific plan.

Root Cause Analysis: A Basic Root Cause Analysis (RCA) involves a closer look at four criteria that may have been a factor in the development of the conditions that led up to an incident. Environment: What About: Site layout, chemicals, temperature, weather, noise, radiation, terrain, vibration, ergonomics, lighting, biological influences, and ventilation. •

Termination activities will concentrate on giving accurate information to people who need it most especially employees, neighborhood, Port Authorities, Govt. Authorities engaged in offsite emergencies, and will begin as soon as the emergency phase of the operation is completed. The termination of emergency will be declared through siren as per the Siren Code defined by industry in case of Level - I and II. For Level-III emergency, termination of emergency will be declared by Port / Govt. Authority through appropriate mode of information transfer to reach each and every one.



The Emergency Response Plan of Project Site will be integrated with that of existing Conex petroleum Refining Coy’s Terminal, wherein Terminal Manager will coordinate with Port/ Govt. Authority for Level III Emergency. •



Emergency Recovery Procedure Business / work activity restoration planning is as important as having an emergency response plan. All the management personnel need to be involved in bringing the operation of Project site to normalcy after an emergency has occurred. This involves an emergency response planning and follows up. The Project Manager will be completely responsible for restoration of site.

After the emergency, the following activities will be carried out in detail by the concerned coordinator. a. b. c. d. e.



Information to statutory authorities, as needed. Incident investigation. Damage assessment. Salvage of Equipments, products, de-contamination, clean up and restoration. A detailed report will be prepared based on the entire experience of the incident, including restorations, limitations and lessons learnt. f. EMERGENCY / FIRE ORGANISATION At Lutech Engineering, Safety, Health and Environment are everyone’s responsibility. As a condition of employment with Lutech Engineering, all employees are accountable to adopt safety as a value and comply with the Best Practices of the highest level of Safety, Health and Environment Standards and Guidelines. Therefore, every staff is part of Emergency Response Plan Organization. Role of key personnel is clearly defined to avoid confusion and to meet the emergency effectively. RESPONE PROCEDURES & MEASURES Containing the consequences of an emergency requires well-planned and documented procedures to ensure prompt response and coordination among various task groups. The following response procedures as prevalent in the location are carried out during emergency. Fire at Jetty end Fire may take place due to leak / rupture of pipeline carrying product in Tanker Discharge line. Fire in Tanker, Exchange pit, hammering of defective valve or use of sparking tool. In the event of such situation following action shall be taken: •



Fire has to be attacked promptly with the nearest available extinguishers before it has a chance to spread. Alarm should be raised.







All unloading operations, if any, should be stopped at once. Pumps have to be turned off, hoses be removed, and all the valves should be closed without much delay.







Tanker Master to be informed for stop pumping







DCP coupled with sand should be discharged to extinguish the fire.







Water monitors / jets need to be used to cool the Tanker and adjacent pipelines, as needed. Apply foam to extinguish the fire







Inform Port Authority, Fire Service / PFSO for assistance







Inform China Union to enable them to take precautionary measures including stopping of operations.







If the pipeline leaks, then foam should be applied gently at the affected area. In the same time, the above steps shall also be taken care of.

All Major, Minor and Near miss incidents will be reported in the quarterly report. Incident Reporting form will be filled up for all Major, Minor and near miss Incidents. Summary report will be enclosed with every quarterly report. Investigations will be carried out for all Major, Minor and Near miss Incidents. Investigation report of all Major incidents will be submitted to PNGRB. An incident will be treated as Major if any of the following occurs; •



Fire for more than 15 minutes







Explosion / Blowout







Fatal Incident.







Loss above USD 5,000.







Cumulative man hours lost more than 200 hrs.







Plant Shutdown / Outage due to the incident







Level III incident



Loss time Incident will be monitored till the affected person joins duty. In case the affected person is yet to join the duty, then the status of report submitted will be preliminary. Final report against the same incident will be sent once he joins duty and the man - hours lost are known. All columns must be filled up. For any additional information use separate sheets as required. Immediate reporting of incident through mail / telephone shall continue as per the prevailing system.
