Dialogue between a therapist and a patient with neck spasm symptoms who wants to attend a physical therapy appointment. Patient



: Assalamu’alaikum,



Pt’s



: Wa’alaikumussalam, good morning sir, welcome to puspa clinics. Please come in.



Patient



: Morning, Thankyou.



Pt’s



: My name is Robby as the physical therapist for this session. Do you have an appointment?



Patient



: Yeah, I have called to make an appointment for today.



Pt’s



: That’s great, I will accommpany you for today’s session. May I know your name sir?



Patient



: My name is Zharif, you can call me Zharif



Pt’s



: How do you feel today sir?



Patient



: Not really good, I have been in pain on my neck and shoulder since three days ago.



Pt’s



: Okay, I would like to ask some general questions to begin, then we will see what could be the problem for your pain. Is that alright?



Patient



: Sure



Pt’s



: Where do you live, sir?



Patient



: I live in jakarta



Pt’s



: Okay, Would you please tell me your date of birth?



Patient



: I was born on July 25th, 2000.



Pt’s



: What do you do for a living?



Patient



: Currently I’m a medical student, hopefully I can be physical therapist in the future.



Pt’s



: Oh so you are physical therapy student. Why came here if you can handle your own physical problem? Hahaha



Patient



: Hahaha, actually I’m in my first year, I still not really understand what is the problem for my condition.



Pt’s



: No problem, sooner or later, I believe you will be good at this.



Patient



: Aamiin, Thakyou.



Pt’s



: Okay, let’s move on. How tall are you, and what is your weight now?



Patient



: I am 181 centimeters tall, and 55 kilograms.



Pt’s



: Please tell me about your family, do you live alone or with your parents?



Patient



: I do live with my parents, but sometimes I live alone in my appartment. But this year I stayed at my parent’s house due to covid 19 pandemic.



Pt’s



: Do your families have history of certain disesase?



Patient



: Yeah, my father had a stroke ten years ago, but he is doing well right now.



Pt’s



: Okay, so, please tell me about your pain. When and how it happened?



Patient



: So, four days ago, I was having busy time to move out all of my stuff to my new room. I lifted a lot of heavy things including sofa, bed, and even cupboard to my new room. Later in the afternoon I felt my shoulder ache a litte, so I applied balsem to my shoulder. The worst time was in the next morning. My neck and my shoulder completely felt so bad. I have trouble moving my head untill now.

: What activities that aggravate your pain?



Patient



: Lifting heavy weight, turning head around



Pt’s



: What activities can eases your pain?



Patient



: the pain gone when I don’t do anything



Pt’s



: Okey, do you take any medication for your pain?



Patient



: No, I haven’t taken any oral medication, just balsem for my shoulder.



Pt’s



: Do you have any other health condition?



Patient



: No, I don’t.



Pt’s



: On a scale from one to ten, how bad is your pain fells like?



Patient



: About 7, It’s really distrubing my daily acitivities.



Pt’s



: Okay, Thankyou sir. After this I will do physical examination, and will provide some exercise to release the pain and the stiffness of your neck.

: Oke, thankyou
