GENERAL ASSESSMENT Nurse: Hello good morning, is there anything I can help you with? Patient: Good morning, I came here to see doctor Nurse: can you wait a minute? I will write down your name Patient: Okay Nurse: Have you been here before? Patient: No, it’s my first time coming here Nurse: Okay, what’s your name? Patient: My name is Nurainin Malik Nurse: How old are you? Patient: I am 19 years old Nurse: Where do you live? Patient: I live in Jl. Rusli Datau, Gorontalo Nurse: Are you married? Patient: No, I’m a student Nurse: Do you have health insurance? Patient: No Nurse: What makes you come here? Patient: I want to go to the doctor to check my skin disease Nurse: Ok, please go to the waiting room. The doctor will come soon to check you Patient: Okay, thank you Nurse: You’re welcome



DIMENSION OF SYMPTOM Nurse: Good morning Tiara, Are you feeling better? Is there any progress with your condition? Patient: Yes, I am feeling better. But sometimes I really feel weary, whereas I feel really healthy before that. Is it normal or not? Nurse: Yes that is really normal. That symptom indicates you will be healthy. May I check your body temperature and blood pressure please? Patient: Yes, of course nurse Nurse: After I checked, your body temperature is 38°C and your blood pressure is 120/80. I can diagnose that your healthy is getting better, your blood pressure has been normal, but your body temperature is still high. You need to have more rest to make it better. Patient: How about the medicane nurse? I have to consume the new medicine or the previous medicine? Nurse: You have still to consume the previous one. I will give the medicine that doctor recommend. By the way, how’s about the service here? Is there any problem about that? Patient: I am satisfied about the service. The nurse is very friendly and the facility is very well.
