Tentang strategi GINA • GINA bukan merupakan sebuah pedoman, namun strategi berbasis bukti yang terintegrasi yang berfokus pada praktek klinis • Rekomendasi yang di lampirkan bukan merupakan jawaban PICOT, namun merupakan bagian dari strategi terintegrasi yang berkaitan dengan: • Tujuan GINA untuk mencegah eksaserbasi dan kematian, juga untuk mengendalikan gejala asma • Pemahaman mengenai penyakit yang mendasari • Kebiasaan manusia (tenaga medis dan pasien) • Implementasi klinis praktis • Variasi global populasi, system kesehatan dan akses kesehatan
Latar belakang perubahan tahun 2019 – risiko asma ringan • Pasien asma ringan memiliki risiko serius efek samping • 30–37% dewasa dengan asma akut • 16% pasien dewasa dengan asma yang mengancam nyawa • 15–20% yang meninggal karena asma
Gejala kurang dari 1x/minggu dalam 3 bulan terakhir (Dusser, Allergy 2007)
• Pencetus eksaserbasi bermacam-macam (virus, serbuk bunga, polusi, ketidakpatuhan pengobatan) • Inhalasi SABA telah menjadi lini pertama asma selama 50 tahun • Terhitung sejak era asma dianggap sebagai penyakit bronkokonstriksi • Kepuasan pasien terhadap SABA disebabkan oleh aksi cepatnya mengurangi gejala. Ketersediaan di IGD dan harga yang realtif murah • Pasien biasanya meyakini bahwa “obat pelega saya sudah bisa mengendalikan asma saya”, sehingga mereka tidak merasa memerlukan terapi tambahan
Latar belakang perubahan tahun 2019 – risiko asma ringan • Penggunaan SABA yang sering dan teratur berkaitan dengan efek samping: • Down regulasi reseptor beta, menurunkan proteksi bronkus, efek rebound hiper reaktifsitas bronkus dan menurunkan respon bronkodilator (Hancox, Respir Med 2000) • Meningkatkan respon alergi, meningkatkan inflamasi eosinifilik saluran napas (Aldridge, AJRCCM 2000)
• Penggunaan SABA yang lebih tinggi berkaitan dengan efek samping klinis berikut: • Penggunaan ≥3 canister per tahun (rata2 1.7 puff/hari) berkaitan dengan risiko perawatan IGD lebih tinggi (Stanford, AAAI 2012) • Penggunaan ≥12 canisters per tahun berkaitan dengan risiko kematian lebih tinggi (Suissa, AJRCCM 1994)
Sejarah 12 tahun dibalik perubahan GINA 2019 • Sejak 2007, GINA telah aktif mencari intervensi asma ringan • Untuk menurunkan eksaserbasi dan kematian akibat asma • Menyampaikan pesan yang konsisten tentang tujuan pengobatan asma, termasuk pencegahan eksaserbasi, di seluruh spektrum keparahan asma • Menghindari ketergantungan SABA pada awal perjalanan penyakit
• GINA menekankan kepatuhan yang rendah sebagai faktor risiko eksaserbasi • Pada penggunaan SABA sebagai pelega, kepatuhan yang buruk terhadap penggunaan pengontrol membuat pasien berisiko hanya menggunakan SABA saja sebagai terapi asma
• Anggota GINA berulang kali mencari dana RCT ICS-formoterol sesuai kebutuhan untuk mengurangi risiko asma ringan • Akhirnya memuncak pada tahun 2014 dengan inisiasi penelitian SYGMA, yang diterbitkan pada 2018 (O’Byrne NEJMed 2018; Bateman NEJMed 2018)

Step 2 – terdapat dua pilihan pengontrol yang direkomendasikan
ICS dosis rendah harian dikombinasikan dengan SABA sesuai kebutuhan • Bukti • Banyak bukti yang dari RCT dan penelitian obsevasional bahwa ICS dosis rendah dapat mengurangi eksaserbasi, rawatan di RS dan kematian e.g. Suissa, NEJMed 2000; Suissa, Thorax 2002; Pauwels, Lancet 2003; O’Byrne, AJRCCM 2001
