PROLOG (Prologue) By. Ny. L usia 5 hari di Ruang Irna Perina RSUA dengan diagnosa hiperbilirubinemia akan dilakukan Discharge Planning. Setelah 5 hari dirawat By. Ny. L diperbolehkan pulang oleh dokter karena kondisinnnya sudah membaik. Oleh karena itu. Karu, PP dan PA di Ruang Irna Perina RSUA akan melaksanakan tindakan Discharge Planning. By. Ny. L age 5 days in the Irna Perina Room with a diagnosis of hyperbilirubinemia, a Discharge Planning will be carried out. After 5 days of being treated, By. Ny..L had to go home by the doctor because her condition had improved. Therefore the NUM, PN and AN in the Irna Perina Room will carry out the Discharge Planning action. TAHAP PERSIAPAN (Preparation Stage) PP (Ners Tantya) menyampaikan kepada KARU (Ners Yuniar) bahwa By. Ny. L sudah diperbolehkan pulang oleh dokter karena kondisi sudah membaik, sehingga akan dilakukan Discharge Planning. The PN (Ners Tantya) tell the NUM (Ners Yuniar) that By. Ny. L has been allowed to go home by the doctor because the condition has improved, so a Discharge Planning will be carried out. PP:Selamat pagi Ners Yuniar. PN: Good morning, Ners Yuniar KARU: Selamat pagi, Ners Tantya. Ada yang ingin disampaikan? NUM: Good morning, Ners Tantya. Do you have anything to say?

PP: Yang pertama tentang memandikan bayi, sebenarnya memandikan bayi cukup dilakukan 2-3 hari sekali untuk bayi yang belum merangkak dan lingkungannya tidak kotor. Bayi hanya perlu dibersihkan pada area wajah, leher, dan bokongnya. Boleh dimandikan setiap hari jika memungkinkan. Dan yang perlu diperhatikan, jangan memandikan bayi sebelum atau sesudah bayi makan karena perut yang tertekan akan membuatnya muntah.
