PURPOSE & SCOPE TUJUAN & RUANG LINGKUP



The purpose of this procedure is to provide the methodology for general and workplace specific induction program as per requirement of Indonesia Government on the OHS Statutory Legislation No. 1 Year 1970 Chapter V Article 9, which include but not subject to the order of the following topics:  Workplace Occupational Health and Safety  Environment  Quality  Administrative & HR Processes  General PT OKI System Requirements This procedure shall be applied for all department within PT OKI. Whenever required, the detail guidelines/ work instruction can be developed further based on this procedure. Tujuan prosedur ini adalah memberikan metodologi Progam Induksi Umum dan tempat kerja khusus berdasarkan Undang-Undang No. 1 Tahun 1970 Tentang Keselamatan Kerja Bab V Pasal 9, yang mencakup namun tidak harus sesuai urutan dari:  Keselamatan Karyawan  Lingkungan Hidup  Mutu  Proses-Proses Administrasi dan HRD  Persyaratan sistem PT OKI secara umum Prosedur ini berlaku untuk semua departemen di bawah PT OKI. Apabila diperlukan, panduan yang lebih detail akan dibuat merujuk kepada prosedur ini.

DEFINITIONS DEFINISI-DEFINISI



General Induction



A general orientation given to all new and transfered employees to PT OKI addressing general Occupational Health, Safety and Environmental, HR & Administrative, QMS, and general PT OKI system requirements aspects of working in office or on site. Suatu orientasi umum yang diberikan pada semua karyawan baru atau pindahan ke PT OKI mencakup kesehatan dan keselamatan kerja umum, lingkungan hidup, HR & Administratif, QMS dan aspek persyaratan sistem PT OKI secara umum mengenai bekerja di kantor atau site..



Long Term Visitor



Visitor who visit PT OKI site more than 5 (five) days. Pengunjung yang mengunjungi site PT OKI lebih dari 5 (lima) hari.



Temporary ID/ Personal Access



A printout copy of form that is used, in case of improper induction scheduling and/or the badge technical printing problems, to keep authorizing personnel who fulfill all requirements prior to issue permanent ID/ Personnel Access.

Short Term Visitor



Visitor who visit PT OKI site within 5 (five) days. Pengunjung yang mengunjungi site PT OKI dalam dari 5 (lima) hari.



Visitor Orientation



A general orientation program that given to Visitor immediately upon arrival to the PT OKI site. Suatu program orientasi umum yang diberikan pada pengunjung segera setelah memasuki site PT OKI.



Workplace Specific Induction Program



A job-site specific orientation program is provided to all new or transfered employees. Suatu program orientasi spesifik di tempat kerja yang diberikan untuk semua karyawan baru dan pindahan.

PT OKI is committed to full compliance with relevant Indonesian Government statutory requirements as detailed in: STATUTORY LEGISLATION NO. 1 YEAR 1970 ABOUT WORK SAFETY CHAPTER V ESTABLISHMENT Article 9 1) The management shall show and explain to every new employee the following: a. The conditions and hazards that can appear in his/ her workplace. b. All safety and protective equipment that shall be use in his/her workplace. c. Personal Protective Equipment for the employee; d. The safe method and desired attitude in doing the job. 2) Management can only put the employee to work after the employee has been trained in all aspects of safe work practices. 3) The management is responsible to train all the employees about prevention of accidents and the eradication of fire hazards. This extends to improving health and safety on site and training the employee in administering basic first aid. 4) Management is responsible to fulfill and obey all the requirements and regulations that are valid for company business and relating to the workplace. PT OKI berkomitmen untuk mematuhi secara penuh persyaratan peraturan Pemerintah Indonesia yang terkait sebagai berikut: UNDANG-UNDANG NO. 1 TAHUN 1970 TENTANG KESELAMATAN KERJA BAB V PEMBINAAN Pasal 9 1) Manajemen wajib menunjukkan dan menjelaskan pada tiap karyawan baru tentang: a. Kondisi-kondisi dan bahaya-bahaya yang dapat timbul di tempat kerjanya. b. Semua pengamanan dan alat-alat perlindungan yang harus digunakan di tempat kerjanya. c. Alat-alat perlindungan diri bagi karyawan yang bersangkutan. d. To attend General Induction programs offered and comply with the requirements presented during inductions. To attend specific workplace induction. Implement all PT OKI requirements and standards at workplace. Menghadiri program Induksi umum yang diberikan dan mematuhi persyaratan yang dikemukakan selama induksi. Menghadiri induksi khusus di tempat kerja. Menerapkan semua persyaratan dan standar PT OKI di tempat kerja.



4.2.2 Supervisory Level (Foreman/Supervisor/Superintendent) Tingkatan Pengawas (Foreman/Supervisor/Superintendent)    



 



To ensure all their new and transferred employees and long term visitor receive and understand the general and site specific induction within 5 (five) days of job placement. To ensure all short term visitor have receive the appropriate visitor orientations prior to visit PT OKI site. To escort his/her short term visitor accordingly. Memastikan semua karyawan baru dan pindahan serta pengunjung jangka panjang mendapat dan memahami induksi umum dan spesifik di tempat kerja dalam 5 (lima) hari setelah penempatan kerja. Memastikan semua pengunjung jangka pendek mendapat orientasi pengunjung yang diperlukan sebelum memasuki site PT OKI. Memandu pengunjung jangka pendek dengan baik.

4.2.5



HRD



   



To coordinate and facilitate General Induction Program. HRD is responsible to make sure all employees have completed the Induction Program. Files employee induction administration Issuing ID/ Personal Access badge



   



Berkoordinasi dan memfasilitasi Program Induksi Umum. HRD bertanggung jawab memastikan semua karyawan telah mengikuti Program Induksi Menyimpan hal-hal berkaitan dengan administrasi induksi Memberikan ID/ Personal Access badge.



4.3        



PROGRAM PROGRAM Once a new person signs off employment agreement, HRD Officer shall arrange the new-hired personnel to attend General Induction Program Employees working for PT OKI shall undergo induction program which will be conducted by PT OKI HSE officer/ coordinator. HSE coordinator shall record employee for induction program on HSE Induction Record Form. (ref.P-OkiPL1-100-15-FRM-0002). On completion of the induction, employee shall fill-up and submit the HSE Induction Declaration Form (ref.P-OkiPL1-100-15-FRM-0001). On completion of the induction program, the new employee shall be escorted by HRD/ HSE officer to the workplace and introduced to his/ her supervisor and colleague. PT OKI HSSE Policy, Company Regulation, and General Safety Regulation will be delivered during Induction Program. Due to specific reason, general safety induction shall be provided as priority whiles the rest to be followed. Sesaat setelah seseorang menandatangani perjanjian kerja dengan PT OKI, HRD harus menyiapkan yang bersangkutan untuk mengikuti program induksi umum. Karyawan yang bekerja untuk PT OKI harus mengikuti program induksi yang akan disampaikan oleh HSE officer/ HSE coordinator PT OKI. HSE coordinator harus mencatat catatan induksi karyawan pada HSE Induction Register Form (POkiPL1-100-15-FRM-0002). Setelah selesai mengikuti induksi, karyawan harus mengisi dan menyerahkan HSE Induction Declaration Form (P-OkiPL1-100-15-FRM-0001). Setelah program induksi selesai, karyawan baru harus dipandu oleh HRD/ HSE officer ke tempat kerja dan diperkenalkan pada supervisor dan koleganya. Kebijakan K3L PT OKI, Peraturan Perusahaan, dan Peraturan K3 Umum akan disampaikan pada saat induksi. Dengan alasan tertentu, induksi keselamatan umum harus diutamakan dan yang lainnya dapat menjadi prioritas kedua ISSUANCE OF EMPLOYEE ID/ PERSONNEL ACCESS BADGE PENERBITAN KARTU IDENTITAS KARYAWAN After carrying out the Inductions with HSE Induction Declaration Form (ref.P-OkiPL1-100-15-FRM0001) completed sign off as evidence, the HRD officer shall give ID/ Personnel Access Badge to the employee. Setelah melaksanakan Induksi dan HSE Induction Declaration Form (ref.P-OkiPL1-100-15-FRM0001) yang lengkap ditandatangani sebagai bukti, HRD harus memberikan kartu ID/Personnel Access kepada karyawan tersebut.. 00 10 of 14 May 21, 2014 Induction to HRD. HSE coordinator atau delegasinya perlu memberitahu daftar personil yang telah menyelesaikan induksinya kepada HRD.



LOSS OF ID/PERSONAL ACCESS AND THE REPLACEMENT KEHILANGAN DAN PENGGANTIAN ID/ PERSONAL ACCESS Should a person misplace his/her ID/ Personnel Access, the lost ID/Personnel Access shall be reported to his/her immediate superior and request a new ID/ Personnel Access to PT OKI HRD for replacement within 3 (three) working days. The person shall be fined for losing his/her ID/ Personnel Access. This fine shall be deposited to HRD for ID/ Personnel Access making purpose. In case position changes, HRD shall withdraw the old ID/Personnel Access and give replacement. HRD and each Workplace Managers/Superintendents shall control the return of ID/Personnel Access from employees, including those of contractor personnel, who move and leave site due to resignation or termination and must report it to PT OKI HRD for appropriate deletions of relevant ID/Personnel Access records for security insurance purposes. Apabila seseorang karyawan kehilangan ID/Personnel Accessnya, maka karyawan tersebut harus segera melapor pada atasan langsungnya dan meminta pengganti ID-Personnel Access kepada HRD dalam jangka waktu 3 (tiga) hari kerja. Personil tersebut dikenakan denda karena menghilangkan ID/Personnel Access. Denda ini akan dimasukkan ke cost center HRD untuk keperluan pembuatan ID/Personnel Access. Jika jabatan karyawan berubah, HRD. harus menarik ID/Personnel Access yang lama sebelum memberikan yang baru. HRD dan masing-masing Manajer/Superintendent di tempat kerja harus memantau pengembalian ID/Personnel Access dari karyawan, termasuk karyawan kontraktor yang mengundurkan diri atau diPHK serta harus melaporkannya ke HRD PT OKI untuk penghapusan catatan ID/Personnel Access terkait jaminan keamanan. GENERAL SAFETY INDUCTION INDUKSI KESELAMATAN UMUM



The general safety induction should be presented by safety expert/ experienced safety staff. Induksi Keselamatan Umum hendaknya diberikan oleh staff yang ahli/berpengalaman di bidang keselamatan kerja. ENVIRONMENTAL INDUCTION INDUKSI LINGKUNGAN HIDUP



The employee shall be given environmental management awareness, work procedures with which the employee will be involved. Karyawan harus diberikan kesadaran akan lingkungan hidup, prosedur kerja di mana karyawan akan terlibat 4.8



QUALITY MANAGEMENT SYSTEM INDUCTION INDUKSI SISTEM MANAJEMEN MUTU



The employee shall be informed of the PT OKI quality management system structure of the relevant work procedures, quality control requirements, and the location/access to quality documentation. For administration staff, he/she also shall be given document control procedure (ref. P-OkiPL1-100-16PRO-0001) and record control procedure (ref. P-OkiPL1-100-16-PRO-0002). Karyawan harus diberitahu mengenai struktur sistem manajemen mutu PT OKI dari prosedur kerja terkait, persyaratan pengendalian mutu dan lokasi/akses untuk dokumentasi mutu. Staff administrasi harus diberikan prosedur pengendalian dokumen (ref. P-OkiPL1-100-16-PRO-0001) dan prosedur pengendalian catatan (ref. P-OkiPL1-100-16-PRO-0002). 4.9

Fire extinguisher location d. Restricted access e. Amenities location f. Induksi spesifik di tempat kerja harus dilaksanakan pada karyawan di tempat kerja khusus sebagai berikut:  Rencana peta lokasi yang menunjukkan: a. Fasilitas pertolongan pertama b. Evakuasi yang Terluka Parah (Medivac) c. Lokasi pemadam kebakaran d. Tempat-tempat tertutup e. Lokasi fasilitas yang tersedia f. Jalur evakuasi dan titik kumpul  Peraturan keselamatan kerja di site  Peraturan lalu lintas di site termasuk transportasi  Peraturan komunikasi termasuk frekuensi radio  Rapat Komite Kesehatan dan Keselamatan dan Rapat Tool Box  Persyaratan khusus Alat Pengaman Diri (APD)  Persyaratan khusus keselamatan kerja 4.10

EVALUATION EVALUASI On completion of the induction program, participants shall complete induction questionnaire to demonstrate his/ her understanding of the program. Induction conductors shall evaluate the questionnaire and provide report (P-OkiPL1-100-01-FRM0006) to HRD and specifically inform incase the participant needs to be re-inducted. Di akhir program induksi, peserta harus mengisi Kuesioner Induksi untuk menunjukkan pemahamannya akan program tersebut.

RECORDS CATATAN HSE Coordinator to maintain induction record (ref.P-OkiPL1-100-15-FRM-0002) of employee based on training result (ref.P-OkiPL1-100-01-FRM-0006) and HRD to files the General Induction Declaration (ref.P-OkiPL1-100-15-FRM-0001) to employee Personal File. HRD shall keep training attendance (ref.P-OkiPL1-100-01-FRM-0004) and training result (ref.POkiPL1-100-01-FRM-0006) for data input and training matrix (ref.P-OkiPL1-100-01-FRM-0008) update. Whenever an employee is transfers from one project to another and/or reemployed within the 12 (twelve) month period, the employee shall only need to be inducted for his/her workplace specific induction. HSE coordinator harus memelihara induction record (ref.P-OkiPL1-100-15-FRM-0002) karyawan berdasarkan hasil pelatihan (ref.P-OkiPL1-100-01-FRM-0006) dan HRD harus menyimpan file General Induction Declaration (ref. P-OkiPL1-100-15-FRM-0001) dalam Personal File karyawan. HRD harus menyimpan training attendance (ref.P-OkiPL1-100-01-FRM-0004) dan training result (ref.P-OkiPL1-100-01-FRM-0006) untuk input data dan mengupdate training matrix (ref. P-OkiPL1100-01-FRM-0008). Ketika seorang karyawan dipindahkan dari satu proyek ke proyek lain dan/atau dipekerjakan lagi dalam waktu 12 (dua belas) bulan, karyawan tersebut hanya perlu mendapat induksi spesifik di tempat kerjanya yang baru.

VISITOR’S ORIENTATION ORIENTASI PENGUNJUNG The short term visitor shall attend orientation program as follows: a. The workplace layout and location of all amenities b. Workplace safety rules c. Access ways, emergency evacuation route and first aid stations location d. Requirements for personal protective equipment e. Identification of hazardous conditions f. Traffic rules The long term visitor shall attend general & workplace specific induction programs. The host shall ensure his/her visitor is equipped with appropriate PPE and comply with PT OKI rules and regulations. Host will give badge to his/her visitor. Pengunjung jangka pendek harus mengikuti program orientasi sebagai berikut: a. Denah tempat kerja dan lokasi semua fasilitas b. Peraturan keselamatan kerja di tempat kerja c. Jalan akses, jalur evakuasi darurat dan lokasi stasium P3K d. Persyaratan Alat Pengaman Diri (APD) e. Identifikasi kondisi berbahaya f. Peraturan lalu lintas Pengunjung jangka panjang harus mengikuti program induksi umum dan induksi tempat kerja spesifik. Penanggung jawab pengunjung harus memastikan pengunjung dilengkapi APD yang sesuai dan mematuhi peraturan PT OKI. Penanggung jawab akan memberi tanda pengenal kepada pengnjung.
