Permission has been received from port authority, if necessary.



SECTION 1 1.



Has all life support equipment to be used, bosun’s chairs, stages, gantlines, safety harnesses, lifelines, Jacob’s Ladder etc., been carefully examined and checked in good condition?



2.



Has the method of lowering and securing tools, equipment and materials been agreed and the ropes, containers, etc., been carefully examined and checked in good condition?



3.



If work is to be carried out around, or near engine room overboard discharges, has the engine room been notified and Duty Engineer shut down, or diverted, the discharges, posted notice in the ECR and tagged the pumps / valves, etc.?



4.



If working is to be carried out around, or near, other discharges, have they been taken care of or shut and tagged, etc.?



5.



If painting or other preparation is to be done, have all necessary precautions been taken to prevent pollution. ?



6.



Have all persons to work over side been checked for wearing safety harness, before going over. ?



7.



Have all persons to work over side been checked for wearing buoyancy aid, etc.?



8.



Lifebuoy, with line, is handy and ready for use.



9.



Supervisor to remain in attendance throughout time and to ensure that safety harness lines are made fast by the persons over side. Supervisor to inform OOW and Duty Engineer when work is completed. Warning notices to have been withdrawn. Tags removed, power supplies re-connected, etc



In the circumstances noted of is considered safe to proceed with the work. Responsible Officer’s name Issued by officer in charge
